• Panorex x-ray
• 2 cleanings with exams, fluoride & bitewing x-rays
• 1 emergency visit + necessary x-rays
• 15% off all treatment (10% off clear aligners)
Kids ($338 annually, a $729 value)
- Panorex x‑ray
- 2-healthy gum cleanings with exams, fluoride and bitewing x‑rays
- An emergency visit & any needed x‑rays
- 15% off ALL treatment received (10% off KOR/Invisalign)
Perio Plan – $574/year (A $1245 Value)
Includes:
• Full-mouth x-rays and/or panorex
• 3 perio maintenance cleanings
• 2 exams + bitewing x-rays
• 1 emergency visit + necessary x-rays
• 15% off all treatment (10% off clear aligners)
The benefits of your membership will last 12-months from the date of purchase. The plan will not automatically renew and must be re-purchased at the end of the 12-month period. The cost of your membership is non-refundable.
